The OSCA on-line application is in PDF format. Users must have Adobe Acrobat Reader to enter information directly into the application.
To obtain the latest FREE version of Adobe Acrobat Reader, visit their website at: http://www.adobe.com/products/acrobat/readstep2.html.
There is no provision to submit this on-line application electronically. Only hard copy application submittals will be accepted with ”original signatures.”
IMPORTANT: You will not be able to save your completed application, unless you have the full version of Adobe Acrobat. You may want to print out the application and complete a draft before entering information electronically and printing out a hard copy for submittal.
