ELAP Forms and Checklists
This page has all ELAP’s forms and checklists in one place for your convenience. Please be sure to visit our other webpages for more information on how and when to use these forms.
The interactive timeline guidance tool captures the common questions regarding when certain items should be completed to maintain a laboratory’s accreditation. Based on a laboratory’s inputted expiration date, ELAP provides recommended dates or timeframes for when best to perform PTs, schedule and complete the on-site assessment, and submit the renewal application. This tool also includes the calculated regulatory deadlines for each item, however, the tool does have some limitations, as it cannot adequately address or provide recommendations for laboratories who have an interim certificate or are located outside of California.
The Technical Notice below identifies allowances granted for drinking water methods. Both methods EPA 524.3 and 524.4 can eliminate the refrigerated autosampler requirement if the laboratory follows guidance and requirements in the Notice.

This document identifies the naming convention used by ELAP and future Water Board databases. While ELAP makes efforts to update all FOAs to match this convention, some may be dictated by state agency requirements:

Assessment checklists are tools provided to help laboratories prepare for their assessment, but they do not cover all regulatory or technical requirements. Laboratories are responsible for compliance with all regulatory and technical requirements even if they are not included in the checklist.
California Regulations TNI Addendum Checklist – for laboratories being assessed to the quality system requirements in CCR § 64802.05(a) and the 2016 TNI Standard - Revision 2.1, Volume 1. This checklist includes CA regulatory requirements not covered by the TNI Standard. NOTE: This checklist does not include the requirements of the 2016 TNI Standard - Revision 2.1. This checklist is intended to be used in conjunction with the 2016 TNI Standard – Revision 2.1 Quality System Checklist – available at www.nelac-institute.org.
